I've been in Sacred Heart Lake for more than an hour, running through every inch of it, and it still refuses to give me credit for exploring it

If it's the one I'm thinking of (in Besieged Farmlands) it's completely mislabeled. Go over to where Zaha the gypsy girl is and go south until you find a toxic lake. That's it.
